Frequently Asked Questions About TRT

How do I know if I have low testosterone?

Symptoms like persistent fatigue, low libido, difficulty building muscle, brain fog, and mood changes are common indicators. But symptoms alone are not enough for a diagnosis.

The Endocrine Society guidelines recommend confirming low testosterone with at least two morning blood draws showing total testosterone below 300 ng/dL, along with evaluation of free testosterone, SHBG, and LH/FSH to determine the underlying cause. Is testosterone replacement therapy safe?

The TRAVERSE trial, published in the New England Journal of Medicine in 2023, was the largest randomized cardiovascular safety study of TRT ever conducted. It found no increased risk of major adverse cardiac events in men with hypogonadism.

That said, TRT does require ongoing monitoring. Your clinician tracks your hematocrit (red blood cell concentration), PSA, estradiol, and metabolic markers at regular intervals to catch and address any changes early.

How soon will I notice results from TRT?

The timeline varies by symptom:

Energy, mood, motivation - often within 3-4 weeks
Libido and sexual function - typically within 6 weeks
Body composition (reduced fat, increased lean muscle) - 12-16 weeks

The Testosterone Trials (NEJM, 2016) documented measurable improvements across all symptom domains within 12 months.

What makes your approach different from online TRT clinics?

Most online TRT clinics prescribe a standard dose of testosterone based on a single lab test. We start with a focused hormone panel covering total and free testosterone, estradiol, SHBG, LH/FSH, PSA, hematocrit, and the supporting markers needed to dose TRT safely.

Low T can also be a downstream signal of insulin resistance, thyroid dysfunction, cortisol dysregulation, or excess aromatase activity. If your symptoms or history point to a wider issue, your clinician can step you up to the Complete Hormone Evaluation, which adds multi-axis hormone, thyroid, adrenal, and cardiometabolic testing on top of the focused panel. Will TRT affect my fertility?

Exogenous testosterone suppresses your body's own production through negative feedback on the hypothalamic-pituitary-gonadal (HPG) axis. This can reduce sperm production.

For men who want to preserve fertility, your provider may include HCG in your protocol to maintain testicular function and spermatogenesis. This is discussed during your evaluation and factored into your treatment plan from the start.

Do I need to be on TRT for life?

Not necessarily. If your low testosterone is primarily driven by modifiable factors like insulin resistance, poor sleep, chronic stress, or thyroid dysfunction, addressing those root causes can sometimes restore your natural production.

For men with primary hypogonadism (testicular failure) or significant age-related decline, ongoing therapy is typically needed. The Asymmetric Health team reassesses your protocol regularly and adjusts based on your labs and how you feel.

What does the evaluation cost and do you take insurance?

The Focused Hormone Evaluation is $299 as a one-time fee. This covers advanced lab work, a 60 minute evaluation with your provider, and your personalized treatment plan.

As a direct primary care practice, Asymmetric Health does not bill insurance for visits, which means no surprise bills, no prior authorizations, and no insurance company deciding what labs you can or cannot run. Lab work through Labcorp may be covered by your insurance separately.

Can I do this via telehealth?

Yes. Our providers see patients via telehealth anywhere in Washington state. Your lab work is completed at any Labcorp location, and your evaluation, follow-ups, and protocol adjustments are all conducted remotely. In-clinic visits are available at our Lacey, WA office for patients who prefer face-to-face care.
